Hospital Discharge Form – Rappresenta la sintesi di una cartella clinica. Raccoglie le informazioni anagrafiche, amministrative e sanitarie riferite al ricovero. Le informazioni raccolte attraverso la SDO comprendono le caratteristiche socio-demografiche del paziente, la diagnosi principale di dimissione e le eventuali diagnosi secondarie, gli eventuali interventi chirurgici e procedure diagnostiche e terapeutiche effettuate, i reparti di ricovero e di dimissione, lo stato del paziente al momento della dimissione e la sua destinazione dopo il ricovero. Assume valenza di carattere medico-legale, incluso il carattere di “atto pubblico, dotato di rilevanza giuridica, la cui corretta compilazione obbliga la responsabilità del medico”. –
